If you move, or choose to transfer your records to another doctor, we will need a record release form signed by you designating where you want your records transferred. You many also designate the information you want transferred, such as 'lab results only'. The forms are available at our office, on the website (under Patient Forms), or from your new doctor.
We will transfer these records by mail or secure fax, depending on how many pages of records that you have.
