We are obligated by law to release information from your medical records to your current health insurance company, should they request it. In other cases, listed below, we will need a signed request from you before releasing medical information:

• Request for records for the purchase of a life insurance policy
• Request for records for an automobile accident
• Request for records for disability
• Request for records for a lawsuit

Requests for records for the above will be initiated by the company or party that needs to obtain the records. For instance, the lawyer who represents you in a lawsuit would request your records. The requester will usually have you sign the release and will send us a copy. However, as noted above, we will not release the records until we get your signed release. Your security is always our first obligation.
