Certain personal and medical information is required in order to give you the best treatment possible, as well as to comply with your insurance company requirements. Clicking on the Health Forms tab at the top of your screen will access the forms you need to complete. Some of these forms require information filled in, while others are to read and acknowledge. After filling out the forms, they will be transmitted to us online.

Two other forms are needed. The records transfer form, as well as the HIPAA Privacy consent form must be printed out and signed. Click on these title below to print out these forms and sign them.
