Your medical record or chart includes many things:

• Your personal information
• Your insurance information
• Your medical history
• Medication records, including pharmacy refills
• Your visits
• Your immunizations
• Results of lab tests, EKG's and other procedures

Due to the personal nature of this information, we rigidly follow the government's laws (HIPAA regulations) regarding who is allowed access to your information. The law allows you, your current medical insurance company, and other doctors that you are seeing to have access to this information. You may also designate a spouse, family members, a guardian, or a friend to have access to this information when you fill out your initial patient forms. You can change those designated by speaking with someone at our office. We do not release information to anyone else without your signed consent.
